Terms & Conditions Agreement

Please read and check each box to acknowledge and agree to the following policies. All boxes must be checked before submitting your application.

I authorize Hoopers to auto-deduct monthly membership fees from the credit card provided. I understand I am not under contract but a monthly membership agreement
I agree to provide at least five (5) days’ notice prior to billing date (1st) for any changes to my payment method. If not there will be a $10 administrative fee.
I understand there are absolutely no refunds after the monthly membership fee has been charged.
I understand that no credits or refunds will be issued for missed practices due to illness, travel, holidays, weather, or any personal conflicts.
Hoopers is not responsible nor do we issue credit for missed practices or games due to weather, holidays or any park events etc. We do give credit in the form of make ups for any missed practices
I understand a $35.00 late fee will be applied if my payment is declined and not resolved within three (3) days. After that an additional $15 administrative will be applied

Non-Response & Payment Policy:

If a member does not respond to payment-related messages within 7 days, Hoopers reserves the right to terminate membership. If member decides to return member must reregister into the program

Membership Pause Policy:

To pause (freeze) a membership, a written request must be submitted before the 1st of the month, including: -Player’s full name & birthday -Month(s) to pause (maximum of 2 months) -Intended month of return
During the pause period, your spot will be held, but access to training sessions and the facility will be temporarily suspended. Billing will automatically resume on the stated return month. No refunds will be issued if a member forgets that billing was set to resume and didn’t want to. We will allow credit in the form of make up classes
If I fail to follow the membership pause policy, I acknowledge no refunds will be provided, and I agree not to dispute or file a chargeback with my credit card company or bank charges from Hoopers

Cancellation Policy:

Members must provide at least five (5) days’ notice prior to their next billing date to cancel a membership. Cancellations made after this window will apply to the following month. No refunds will be issued for missed sessions or late cancellations. Reserved spots and access will be forfeited once a membership is canceled.
If I fail to follow the cancellation policy, I acknowledge no refunds will be provided, and I agree not to dispute or file a chargeback with my credit card company or bank charges from Hoopers
I agree not to initiate a credit card chargeback for any reason that violates this agreement. I understand that doing so may result in additional fees and/or legal action.

Liability Waiver: *

1. The risk of injury to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of injury does exist; and,

2. I FOR MYSELF, SPOUSE, AND CHILD/WARD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child/ward’s participation; and, 3.

I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child/ward’s readiness for participation and bring such attention to the nearest official immediately; and,

4. I for myself, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS HOOPERS; its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY,DISABILITY, DEATH, or loss or damage to person or property incident to my child/ward’s involvement or participation in these programs, WHETHER ARISING FROM NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

5. I, for myself, my spouse, my child’s/ward, and on behalf of my/our heirs, assigns, personal representative and next of kin. HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releases from any and all liabilities incident to my child’s/ward involvement or participation in these programs, EVEN IF ARISING FROM NEGLIGENCE, to the fullest extent permitted by law.

6. I understand all pictures taken of my child by Hoopers, may be used for marketing purposes. Unless written notice is provided

Parent & Player Conduct Policy:

At Hoopers we are committed to providing a positive, respectful, and growth-oriented environment for all players, families, and staff. By participating in this program, both players and parents/guardians agree to: -Treat coaches, staff, teammates, referees, and opponents with respect at all times -Demonstrate good sportsmanship, teamwork, and a positive attitude -Avoid disruptive, disrespectful, or aggressive behavior—both on and off the court -Communicate concerns respectfully and at appropriate times (not during games or practice)
I have read, understood, and agree to all the policies stated above.
